When all my units says it has 100% efficiency but the icon says otherwise why? (except the expenses cause building)

You have eight gyms in the same city, with the same specialisation. This is bringing the efficiency of each unit down. In general, you should only have one subdivision of each type in a city. Different subdivision types will not conflict with each other.
